Enroll for family camp

To sign up for Family Camp 2009, please fill out the enrollment form below, and either make a credit card payment on the page after you click 'submit,' call us with a credit card number, or mail in your deposit. I/we would like to stay in a heated cabin with bathrooom in the building. I/we would like to stay in a summer cabin with bathroom nearby. 25% discount applies. (Available only in Aug.) I/we would like to stay in my/our own tent or RV with a bathroom nearby. 50% discount applies.
I hereby apply for enrollment for Family Camp 2009 at Crystal Lake Camps. I will make a deposit of $50 per person which I understand will be applied to the camp fee and is not returnable unless cancellation is received prior to May 1, 2009. I agree to pay the balance of the camp fee on or before May 15 for the Memorial Day Weekend and on or before June 1, 2009 for the August Session. I understand that if unenrollment occurs within 4 weeks of the start of the camp session, camp fees will not be refunded but can be applied to the following summer. Signature: Date:
